Only one captain from each team needs to fill out the nominations form. Below—You'll see boxes for the offensive and defensive positions. The captain's need only write in ONE player for each spot. The only caveat is that it CAN'T be someone from your own team. After we receive those submissions, we will send a ballot to select the players for each team.
